Office Policies and Forms
As is customary when seeing a health care professional for the first time, there is some information I need about you and some forms for you to complete. You can either download the forms and bring them completed to our first session or you can arrive about 30 minutes early to your first appointment and fill them out then. If you would like to fill them out the day of your appointment, please let me know so that I can have them ready for you.
